Notice Regarding Availability of Form 1095-C
Kinston Community Health Center, Inc. is providing this notice in accordance with applicable IRS guidance regarding Form 1095-C (Employer-Provided Health Insurance Offer and Coverage).
For the 2025 tax year, eligible individuals may request a copy of their Form 1095-C.
A copy of Form 1095-C may be requested by contacting:

Requests may be made by email or in writing.
Copies of Form 1095-C will be provided:
- By U.S. Mail to the address on file, or
- For in-person pickup at Kinston Community Health Center
Forms will not be furnished electronically.
