New patients must meet with a Patient Access Representative to complete the registration paperwork or complete the forms online via our website. Patients must bring their picture ID, insurance information if applicable, and any patient medical records they may have.
To apply for the Slide Fee Discount Program you will need to bring the following:
Identification for all those in the household and current household income. A recent copy of the 1040 Tax forms are preferred, however patients should contact the center to inquire about other acceptable forms of income documentation.
