This Privacy Notice is being provided to you as a requirement of federal law, the Health Insurance Portability and Accountability Act (HIPPA). This privacy notice describes how we may use and disclose your protected health information(PHI) to carry out treatment, payment or healthcare operations(TPO) and for other purposes permitted by law. Protected healthcare information means any written, electronic and oral health information and demographic data that can be used to identify you.
Stephanie Taylor, RN, was raised in Eastern North Carolina. She started at the Health Center as a Dental Assistant student while at Wayne Community College. Upon graduating as a Dental Assistant II she became employed at KCHC in October 2006. In 2011, she obtained an Associates Degree in Nursing at Lenoir Community College. She returned to KCHC as a Registered Nurse full time in May 2015 and in January 2016 became the Compliance and Risk Management Coordinator.
Tracie Zeagler, RN-BSN, studied Nursing at Lenoir Community College and graduated in May 1993. She gained nursing experience working in various pediatric settings, medical-surgical post-op, critical care, and staff development before taking a hiatus from nursing to raise her 5 sons. In August of 2013, Tracie returned back to nursing school and graduated Dec 2014 with a RN-BSN degree. She began working at KCHC in October 2013 as a triage nurse and became the Quality Improvement/Quality Assurance Coordinator in April 2016. She is married and resides in Deep Run.
